What info did you have to include on your request if you mail it out, aside from your name and the exam #? Thanks!

This is complete bullsh*t. I mailed it out first thing Monday morning still haven't received it. I was waiting for my mailman today like a junky online at the crack spot. DCAS can suck my little wht Irish pecker. I put everything that I felt was applicable. Name/social/tax etc etc.

I sent for it Thursday. I did not get to finish and I'm going nuts. Reading all these posts gives me such an anxiety that I didn't have an answer key to take with me. Nevertheless, did u make sure to enter school, room and seat #? I asked a few times if they had everything they needed. I asked if I could just put my last four social and she was like nope. So I chanced a potential identity theft leaving that paper with her with my full social but I rather have peace of mind and just get the damn thing so I could send it to Gamblor.

I went on Monday to DCAS And filled everything on the form. I received it yesterday. Just in time to ruin my weekend!

I mailed my request out first thing Monday... Haven't received it yet... Not sure if I made a mistake doing so... Another thing to stress out about until the 5th week...
