I find it absurd that we are asked to do this in the first place. Why can't DCAS or the FIT unit get it straight without us.

Just went down to DCAS. Surprisingly, the guy I spoke to was very nice and articulate. He also told me that DCAS was totally stymied by the whole fitness point concept and that they might not be able to fix it. He also apologized like three times. I filled out the form. We'll see.
