This may be a stupid question but do medals give you extra points on the exam? If so, is it automatic or you have to submit some request form?

Medals give you extra points. Not much. Youre better off doing the run and the jst. Obviously you have to write up the medals to receive them but after you do they are automatically added to the test you take. That being said they are only added if you pass. You cant get a 69 and ask to use the medals to give you a passing score.

 If your score is higher than it should be, you got the points. If not. You didnt.

Mine were accurate. I believe it was printed on the card they send to your home.
