 I'm going to copy someone's from work probably. I asked Yano and he said we could call DCAS but that seems like more of a pain in the butt.

Yes god help this dept, you didn't know to ask for the sheet! Did you know to bring #2 pencils? I remember taking my first civil service exam! Jeez

lmao, i think you're all still OK...it was a standard form with none of your info on it, it wasn't just for you, im sure you can reach out to DCAS and pick up a copy, or like another poster suggested, use a photocopy and mail it in
