So the CEA only covers up to 1 star chief, meaning you get no union protection past that hence why some chiefs get forced out. But what about the other stuff that comes with a union? Dental, vision, etc? Does the city provide that for them or are they on their own for that?

Only captain to a one-star chief is covered under CEA. 2 star and above are considered city managers and the city covers their benefits.

I was under this impression as well. As far as I know, any rank above Captain is acting in designation, and still falls under CEA.
